Who's responsible when MS systems screw up? Anybody who would ask you that should be directed to call MS on a difficult problem and see what happens when some pimply kid tells them to reinstall everything.

Actually, it depends on who you are. When I was in the Exchange project there was an entire group that just did hot-fixes for important companies that found problems in shipped versions of Exchange. They also did general bug fixes that, with the hot-fixes, were released as service packs.
